[h=2]Proof of identity[/h] Either your Passport or National Identity Card are acceptable. You must provide a full clear colour or black and white certified copy of your Passport or National Identity Card. This document should be current, valid (i.e. not out of date) and your certified photocopy must show your name, your photograph, the Passport or National Identity Card number, your date and place of birth, the issue and expiry dates. If you supply a certified copy of your National Identity Card, this must be photocopied on both sides.
[h=2]Proof of address[/h] You must provide one of the following original documents dated within the last 3 months:
    • Gas bill
    • Most recent bank or credit card statement (Store cards are not accepted)
    • Water bill (last 12 months)
    • Electric bill
    • Telephone bill (land line only)
    • Mortgage statement
    • Driving Licence or National Identity Card
    • Local Authority or Council Tax bill (last 12 months)
    • Sky or Cable TV bill
A PO Box address can only be used as a correspondence address and verification documents of the PO Box address will not suffice unless they also make reference to the residential address e.g. a utility bill that quotes the building at which the utility is being used.
Please note that your statements must be full statements showing your bank or credit card provider’s details and your name and address.
Please note that the documentation for proof of your identity and residential address must differ, therefore a National Identity Card, for example, cannot be accepted for two validations
[h=2]Existing bank statements[/h] We will require a certified copy of the most recent month’s main bank account statements. Temporary Transaction lists and eStatements are not acceptable.
[h=2]Certification of documents[/h] The following people can certify photocopies of your documents:
    • Solicitor, Advocate, Lawyer or Notary Public who is a member of a recognised professional body
    • Embassy or Consular Official
    • Actuary who is a member of a recognised professional body
    • Accountant or Tax Advisor who is a member of a recognised professional body

    Cost of Life Insurance
    http://costoflifeinsurance.info
Cost of life insurance ultimately depends on many factors including your age, gender, weight, if you’re a smoker, the amount
you wish to take out, the period you plan on taking it out for, whether you’re a healthy weight, have an illness (terminal or not)
and whether or not you work in a hazardous environment.
